How to make a referral
A referral from either a specialist or general practitioner (GP) is required prior to an initial consultation. We would prefer to have as much information as possible prior to the initial consultation, so please include all relevant pathology reports, operative notes and x-rays or x-ray reports.
Referrals can be sent by fax to the centre:
Chemotherapy and Haematology referrals – Fax: (03) 4215 2743
Radiotherapy referrals – Fax: (03) 4215 2836
Please Include in the referral:
- urgent or nonurgent
- outline of problem
- test results
- contact details
- public or private patient

Private referrals can also be made to the centre. A list of doctors, tumour streams and contact numbers are available here.
Referral form
A referral form can be downloaded for those Doctors with Zedmed, Medical Director or Best Practice databases. The GP referral template can be downloaded into your practice management software, allowing you to autofill your patient’s data. These forms can then be printed and faxed to the centre.
